Donor Specifications:           

The scholarship is intended for students attending UA, ASU, and NAU who are involved with their community and/or school and have overcome obstacles in their life presented by a spinal disorder.  Each school will select their top 3 finalists to be reviewed by the Sonoran Spine Foundation Scholarship Committee.

Eligibility:      

U.S. Citizen

Admitted as a full-time student (12 units for undergraduates and 9 units for graduates) in any major

Have a diagnosed spinal disorder

Involvement in community and/or school activities

Demonstrate financial need be filing a Free Application for Federal Student Aid (FAFSA)

Application Submission:

Complete the application by answering each question. Be sure to adhere to word and/or page limits specified. Also include your physician’s statement. Click the orange “UA Sonoran Spine Foundation” link to download the template. When completed, upload your document to your DOCS tab at the top of your page. Then, you will be able to upload your documents and submit the application.
